For surfaces similar to this, it is best to replace sub-grade concrete prior to attempting to tackle the work with a fracture reducer.

Instead of pouring the concrete back in the hole, then use a trowel to smooth the face of the holes till you get a coarse dust such as material. Next, apply the concrete dust mixture all the way round the edges of the damaged region, smoothing it out as you go. Finally, apply one more layer of this compactable material, smoothing out any bubbles that may form while the mix sets.
